Introduction - Thank you for choosing Four Paws With Meg. These terms and conditions outline the agreement between you (the client) and Four Paws With Meg (the provider) for pet care services. By booking services, you acknowledge and agree to these terms.

Terms and conditions

• I hereby authorise the registered veterinary practice I have stated above to treat any of my pets as listed in my form I have completed and I accept full responsibility for all fees and charges incurred in the treatment of any of my pets at the veterinary practice.

• I also accept full responsibility for all fees and charges incurred with Four Paws With Meg in regard to transportation and attendance fees which are outlined in the price list provided to me. I understand that it is my responsibility to raise any questions regarding these costs to ensure full understanding. Four Paws With Meg will not be held liable for any misunderstandings where no prior contact was made by myself to clarify the charges related to this service.

• I authorise Four Paws With Meg to transport my pet/s to and from the veterinary practice.

• If I cannot be reached in case of an emergency, I give permission for Four Paws With Meg to act on my behalf to authorise any emergency first aid treatment up to £500.

• The provider is not responsible for pre-existing health conditions or sudden illness in my pet/s.

• I give permission for Four Paws With Meg to share my insurance policy details with the veterinary practice listed above.

• I will assume full responsibility upon my return for payment and/or reimbursement for any veterinary fees as well as fees incurred in relation to transportation/attendance by Four Paws With Meg. 

I confirm upon submission of any consent form that my above responses to the questions are up to date and correct and that I agree to this statement. 

I further confirm that I agree to all terms and conditions applicable to the Veterinary Release Form, that are set out in the General Terms and Conditions.

Failure to notify Four Paws With Meg in writing of any objection to these terms prior to the commencement of your booking shall be deemed evidence that you read, understood, and accepted the General Terms and Conditions and the Veterinary Release Terms and Conditions in full.
